Subscription Membership

Information

The Subscription Membership program offered by Westerly Ambulance entitles subscribers (who have paid their annual subscription fee), as well as family members listed on the subscriber's application, to ambulance service rendered by the Westerly Ambulance Corps in accordance with the Rules and Regulations listed on Page 2 of the subscription application (which can be downloaded below).

Application

Download the 2008 Subscription Membership Application. Once completed, return the application either in person or via mail to 30 Chestnut Street, Westerly, RI 02891 along with a check or money order, in the amount of $35.00, made payable to the Westerly Ambulance Corps, Inc.
